The game takes place somewhere between the years 2015 and 2026 after an in-universe event called the "Contract Wars". Escape from Tarkov is a prequel to the game "Russia 2028" which was a conceptual prototype of a game that was deemed too ambitious.

Set in the fictional Norvinsk region located on the frontier between Russia and Europe, the metropolis of Tarkov was plunged into anarchy thanks to the Contract Wars. What was once a bustling city, is now roamed by rivalling scavenger gangs , former private military forces and black ops units .
The story is framed in a perfect way where Russia's actions towards Tarkov and USEC can't be viewed as immoral. The bad guys in this story is the entire West - the UN, American PMCs, Western biotechnology companies, etc.

Tarkov is sealed off by UN Peacekeepers and the Russian military, supply chains are cut, communication with operational command is lost, and in these conditions everyone has to make their own choices of what to do and how to get out of the chaos-ridden metropolis.
