You will use these entry codes to access the pre-1916 Illinois Statewide Death Index Data Entry Form. Go to the Login Screen by choosing Begin Death Data Entry. Choose the Enter button to enter the Archives Volunteer System. Enter your User ID and Password and then choose the Submit button. Choose the New Death Entry Button to begin data entry.
