- Background on the Physician Fee Schedule. Since 1992, Medicare payment has been made under the PFS for the services of physicians and other billing professionals.
- Evaluation and Management (E/M) Visits. CMS is engaged in an ongoing review of payment for E/M visit code sets. For CY 2022, we finalized several policies that take into account the recent changes to E/M visit codes, as explained in the AMA CPT Codebook, which took effect January 1, 2021.
- Critical Care Services. For critical care services, we are refining our longstanding policies, establishing that: Critical care services are defined in the CPT Codebook prefatory language for the code set.
- Therapy Services. CMS is completing implementation of section 53107 of the Bipartisan Budget Act of 2018, which requires CMS, through the use of new modifiers (CQ and CO), to identify and make payment at 85 percent of the otherwise applicable Part B payment amount for physical therapy and occupational therapy services furnished in whole or in part by physical therapist assistants (PTAs) and occupational therapy assistants (OTAs) ─ when they are appropriately supervised by a physical therapist (PT) or occupational therapist (OT), respectively ─ for dates of service on and after January 1, 2022.
For the 2022 PFS, the RUC submitted 185 recommendations for individual CPT codes. CMS implemented the recommended work values for 77% of these services and nearly all of the direct practice expense recommendations. CY 2022 will be the final year of transition to the new CMS prices for medical supplies and equipment.

Under the rule, the conversion factor will decrease by $1.30 on Jan. 1, 2022, going from $34.89 to $33.59.

The Calendar Year (CY) 2022 Medicare Physician Fee Schedule final rule was released on November 2, 2021 by the Centers for Medicare & Medicaid Services (CMS). The annual payment rule sets policy for physicians participating in the Medicare program and makes updates to the Quality Payment Program. AAOS submitted formal comments to CMS in September.
