The way combat is described in the rules is as if you’re going to roll each attack one at a time (instead of the more common practice of rolling all your bolter shots together, for instance), so when it says “re-roll the attack roll” in the rules, it means the dice that missed.

The effect of a modifier is inversely proportional to the probability of the roll; the worse your chances of success, the greater the impact of a modifier to that roll.
Being able to re-roll all your hit or wound rolls doesn't mean that you have to re-roll all of them, you can choose on a dice-by-dice basis. The only exception is that rolls that require 2 (or sometimes 3) dice like charges or psychic tests do require you to re-roll both dice.
