Using the Mark causes some discomfort for a bearer (itching, burning from inside), but it might apply to the Outsider as well - he seems to be a "power conduit", linking a bearer to energy from the Void (maybe that's why the Outsider chose only eight people in the world, as he couldn't sustain more).
