There can be different commercials as some networks allow the carrier (YTTV, Comcast, Directv, etc) to insert commercials but the time allowance isn't longer. If you're watching something recorded, all bets are off. On some channels, or all (??), YTTV can and does add their own ad(s) at the start.

When it records, you can get fast forward without ads. VoD comes right from the studio and the ads are mandated, this is true of YTTV competitors. Otherwise, if you are binging a back catalog of a show on VOD that doesn't air any more, the only option is to see if another streaming service offers it commercial free.
