Oct 1, 2011 · The screening tests most commonly used to diagnose HAC in the diabetic patient are the low dose dexamethasone suppression (LDDS) test and the ACTH stimulation test. Of these tests, the LDDS has the greatest sensitivity (likelihood that a negative test result is found in animals without the disease) of approximately 90%.
In dogs with naturally occurring HAC an exaggerated response to ACTH administration is accepted (post-ACTH cortisol level greater than 600 mmol/L). The sensitivity of the ACTH stimulation test for diagnosing HAC is 85 per cent and the specificity 90 per cent.

Feb 9, 2024 · The ACTH stimulation test relies on blood samples taken pre- and post-dosing with an ACTH analogue to assess adrenocortical reserve. This is the gold standard for diagnosing iatrogenic Cushing’s syndrome, in which plasma cortisol will not rise post-ACTH administration.
Dogs with hyperadrenocorticism theoretically have an exaggerated response to ACTH. The absolute post-ACTH cortisol concentration is most frequently used to assess the response during an ACTH stimulation test.
A third screening test for HAC is the ACTH Stimulation Test. It is more specific in animals with concurrent disease and recommended for use in dogs with concurrent diabetes mellitus to decrease the possibility of a false positive response.
