1. This report presents key information from 124 DHRs which were reviewed by the Home Office quality assurance process for the 12 months from October 2019. 2. Close to three quarters of these are deaths which occurred in 2017 or 2018. 3. 1. Sixty-one percent of victims had a vulnerability, with 34% having one vulnerability and 27% having more than one. 2.
