Oct 4, 2014 · Happened to me once after doing a ledge kill on the ledge of a cliff. I thought the uruk was too far away and I'd need to distract him closer, but he turned red, so i attacked. After the animation I was stuck inside that cliff. Had to fast travel out and restart the mission. Went to the same exact ledge to try it again, but the uruk was gone.
