You will need to: 1. Fill in a registration form online - diabeticretinascreen.ie 2. Print a copy of the form and ask your GP to sign it 3. Send the signed form to our freepost address
Your details may already be on the Diabetic RetinaScreen register. If they are, you may get a letter inviting you to consent to the programme. DiabeticRetina Screen has a list of people with diabetes.
