Inspector Chan Kwok-Wing is considered one of the most respected members on the Hong Kong Police Force. A year prior to the events of the film, he helps resolve a hostage situation by disguising himself as a reporter, before subduing the hostage taker with only his hands. He receives praise and respect from his colleagues and the public for his heroic actions which led to the female hostage being saved and her captor being arrested and taken into custody.
Shortly after this incident, a gang of youths decides to rob The Bank of Asia, and while they do so, they encourage the bank teller to call the police. They are confronted by a large number of police officers outside the Legislative Building, and it is revealed that the gang plays a sadistic game in which they are rewarded money for shooting at police officers with automatic rifles before making their escape.
After their hideout is revealed, Inspector Chan Kwok-Wing and his squad are called to arrest the gang. However, once they begin their raid, things quickly start going south for his team. Unbeknownst to them, the hideout is rigged with various traps. One by one, Chan's entire team falls into the various different traps, from trapdoors to mazes. Chan himself is spared, however, and later finds his men hanging from a high ceiling in a large warehouse. There, he also meets the gang, before they challenge him to various training skills taught to police officers, such as hand-to-hand combat and pistol assembly. Under mental pressure, Chan loses all of the challenges, and his entire team is ruthlessly slaughtered. The gang the blows up the warehouse with explosives.
Chan takes a year-long leave from the police force due to the trauma that he had endured, becomes nothing more than a shadow of his former self, drinking heavily to drown his sorrows and guilt. However, a young man, Frank Cheng, meets up with him, claiming to be PC 138, the brother of a deceased member of Chan's team. He shows a desire to help Chan capture those who had slaughtered his colleagues. While Chan is initially reluctant, he eventually agrees, and together, the duo visits Sam Wong, a former Hong Kong police officer who had confronted the gang the night of their robbery of The Bank of Asia. Wong relays information to the duo, informing them that one of the robbers is a female and that they enjoy X-Games.
Chan and Frank are followed by Commander Chiu and other policemen as they search for the gang members at an upcoming X-Games event. Sam was forced to help out with the investigation. At the event, held on the rooftop of a skyscraper, Frank and Chan manage to locate one of the gang members, Fire, while Sam and the police interrupt the event and single out the female robber, Sue. Before they can apprehend them, Fire fatally wounds Sam, and the two escape. Sam confesses to Chan that he was blackmailed by the gang to aid them in ambushing Chan's team, and that he alone is responsible for the death of Chan's colleagues.
Frank Cheng and Inspector Chan proceed to chase Sue and Fire, scaling down the building on a tightrope. To distract Chan, Fire shoots and kills a bus driver, sending the bus careening out of control. Chan and Frank manage to save the bus just before it falls into Victoria Harbor. Chan's former superior, Inspector Tai, reveals that he had never assigned him a partner. Chan angrily confronts Frank, who is neither a cop nor a kin of deceased squad member. Frank admits that even though he wanted to be a cop, he failed the exam. Chan is convinced that Frank is sincere, and the two are briefed by Frank's friend, Officer Sa Sa, that the gang members all come from wealthy families and the leader, Joe, is actually the son of the police chief. The gang has also created online video games based on their raids.
