Not much has changed for Will and Grace - or if it has, it wasn't mentioned in the premiere. Grace still has her interior design business, which appears to still be doing well (especially with Karen's rich friends on the client list). She's also added an employee, one who doesn't bring in rich clients, but who also doesn't spend their days fondling...
Karen has also done absoluteley nothing new in the eleven year gap - or so it seems. She's still tossing back pills and martinis, and is still rich and married to a living Stan. There are still quite a few questions that could be answered about the last eleven years of Will and Grace's history, and how it fits into season 8 of the show. Not just the finale, but large chunks of the show seem to have been wiped from continuity, including Grace's pregnancy (which was the tipping point for Will and Vince's relationship to end)...
