When Cobb, Eames, and Saito visit Youssef for the first time, Cobb goes under. He never spins his top after that to confirm he's in reality because Saito interrupts him. Everything before - the Cobol job, Ariadne, etc. - was real. Everything after is part of that dream. That's what I got, feel free to tear apart or add.
