As the name suggests, evidence-based medicine (EBM), is about finding evidence and using that evidence to make clinical decisions. A cornerstone of EBM is the hierarchical system of classifying evidence. This hierarchy is known as the levels of evidence. Physicians are encouraged to find the highest level of evidence to answer clinical questions.

While most ranking schemes consider strength of evidence for therapeutic effects and harms, the OCEBM system allows clinicians and patients to appraise evidence for prevalence, accuracy of diagnostic tests, prognosis, therapeutic effects, rare harms, common harms, and usefulness of (early) screening.

The production of an evidence-based guideline requires a systematic review and critical appraisal of the literature relevant to the scope of the guideline, as guideline recommendations are graded on the strength of evidence on which they are based.
Objective: Grading of recommendations, assessment, development, and evalua- tions (GRADE) is arguably the most widely used method for appraising studies to be included in systematic reviews and guidelines.
GRADE provides a framework for specifying health care questions, choosing outcomes of interest and rating their importance, evaluating the available evidence, and bringing together the evidence with considerations of values and preferences of patients and society to arrive at recommendations.
