Definition of Community Rehabilitation For the purposes of this guidance, community rehabilitation is defined as all rehabilitation delivered to a patient in any setting outside a hospital. This description includes all rehabilitation delivered by Social Services and any rehabilitation provided in community hospitals or care homes.
