The coronavirus (COVID-19) vaccination consent form and letter templates are available in different software versions and can be downloaded. The letter templates can be adapted to suit the needs of local healthcare teams.
Copies of the adult consent form (PDF version) are available to order using product code COV2020376V2.

All CVS Pharmacy locations are expected to have the vaccination in stock by early next week. Appointments for individuals aged five and above are made available at CVS.com and via the CVS Pharmacy app, and walk-ins will be accepted as individual locations receive the vaccines.
Because the Pfizer-BioNTech vaccine is the only FDA-authorized COVID-19 vaccine for adolescents in this age group, patients are encouraged to schedule an appointment online at cvs.com or through the CVS Pharmacy app to ensure availability. Walk-ins are also accepted.
