I noticed the same thing, but I personally thought it was refreshingly realistic. A lot of the time, dudes like this don't get any deus ex machina, or redemption arc. They just end up dead. I think you could actually make a case for the ending absolving him a little bit, though.

The Voices is a dark masterpiece. This movie is amazing. Ryan reynolds does a flawless job of portraying a mentally ill man living in an abandoned bowling alley who stops taking his medication, and then goes down a mind bending rabbit hole of murder.

A dark comedy about a murderous psychopath who hears voices, played by Ryan Reynolds. It's like a rehearsal for Deadpool. I really loved this movie, it's completely different than anything I've watched so far.
