Dr. Miguel "Mike" Cruz joins All Saints as chief administrator in Season Four when the company Quantum Bay buys out the hospital. Bobby Cannavale plays Dr. Mike.
(WARNING: Spoilers) He has been divorced multiple times, and has full custody of his son. He suffers from panic attacks, which he insists is asthma. He tries to help his son with his drug addiction by bringing him to rehab a total of five times. Officious and demanding, he is not well liked in the ER, but occasionally he shows some sensitivity to his staff. He frequently butts heads with other members of the staff, including Jackie, who challenges his authority and threatens him because of her extraordinary expertise. In mid-season, a significant "reveal" occurs when we learn that Dr. Mike is the father of Jackie’s friend, eighteen year-old, Charlie, from rehab. Charlie makes his father promise not to fire Jackie. The next day, in a heavy-handed act of asserting his administrative power, he fires Gloria and Eddie, for enabling Jackie's drug addiction, while honoring his promise to his son. Later, after suffering from a cardiac incident he discovers that Jackie has hired seven temporary nurses which infuriates Cruz. He provokes Jackie in a public scene and then fires Jackie for insubordination. Shortly after, when Jackie returns to the hospital for the birth of O’Hara’s son, Cruz intercepts Jackie and attempts to force her to leave just as Charlie is brought into the ER. Charlie has overdosed which leaves Mike devastated. In a state of panic, he asks for Jackie's help. They try to revive Charlie but he eventually succumbs. Jackie leaves Cruz to mourn privately which ends the fourth season.
We see Mike Cruz again at the beginning of season five after it becomes known that Jackie has been leaving voicemails on Charlie’s old number. He calls Jackie while she is on a date and asks to see her. They go back to his apartment where he confesses that he is having a hard time with his son’s death. Jackie holds him as he cries. They then kiss and have sex. Jackie gets a phone call and has to leave abruptly. The next day he returns to the hospital for the exit interview he’s been putting off. He confesses to Jackie that while he had to come back for his exit interview he also came back to see her. He tells her that he thinks they are more than a one time thing. Later, Jackie returns to his apartment. When he answers the door, she kisses him. After they break apart he asks her if that was goodbye. She says yes and leaves to return to the man she is dating at the time.
